Applications for leave for periods exceeding 4 days shall be submitted in writing through the officer in charge of the Fire Station to the Chief Fire Officer and shall state full particulars of his or her reason for which leave is required the address of the applicant when on leave and the date of the last leave granted and in the case of sickness must be accompanied by a certificate from a government medical officer recommending that it be granted.
