5. Application for licence

(1) An application for a licence for the operation of a body art facility must be made to the Board.

(2) An application under subregulation (1) must —
1. (a) be in the form set out in Form 1 of Schedule 1;
1. (b) include a plan and other particulars of the building and equipment to be used in the body art facility;
1. (c) be accompanied by the application fee specified in Schedule 2.
