(1) If a person is not satisfied with a decision of the Licensing Authority under this Act, the person may appeal against the decision by notice in writing to the Appeal Board, setting out the grounds of the appeal and shall serve a copy of the notice on the Authority.
(2) The notice shall be in the prescribed form and lodged with the Appeal Board within 30 days of the decision or within such longer period allowed by the Board, and the notice shall set out the reasons for the appeal.
