514. Form of policy
Subject to the provision of any Act, contract of life insurance is inadmissible unless it is embodies in a policy. Such policy must be signed by or on behalf of the insurer, and must contain—
(1) the name or sufficient designation of the person;
(2) a declaration of the amount of the insurance, of the amount or rate of premium, and of the commencement and duration of the risk.
