(1) The Director of Finance and Planning shall cause to be prepared and issued such forms of declarations as may be required for carrying out the purposes of this and the next succeeding section, as may be approved by the Governor General.
(2) Every person applying to take out any licence under this Act may be required by the Director of Finance and Planning or a revenue officer to fill up and sign a declaration or declarations in such form as shall be prescribed, wherein shall be stated among other things—
(a) whether such person keeps any horse, mule, carriage or cart, and if he or she does, the number, particulars and description thereof;
(b) in respect of which of such animals or articles he or she has taken out a licence and the date, number and place of issue of such licence; and
(c) if a licence is unnecessary in respect of any such animal or article, such person shall also state in what respect and upon what ground the licence is unnecessary.
