A person qualified to complain under section 8 may complain on grounds that a health service provider has acted unreasonably—
(a) by not providing a health service to a user;
(b) in the manner of providing a health service to a user;
(c) by denying or restricting the user's access to records kept by the health service provider and relating to a user;
(d) in disclosing a user's health records; or
(e) by not properly investigating, or not taking proper action upon, a complaint made to a health service provider by a user about a health service provider's action which is of a kind listed in paragraph (a) to (d).
