4. Certificate of incorporation

(1) A certificate of incorporation under section 6 of the Act shall be in the form set out in Form 2 of Schedule 1.

(2) An application for a duplicate of certificate of incorporation under section 6 of the Act shall be in the form set out in Form 3 of Schedule 1.
